At Olidia Care, our vision is to develop a family-centered approach to home health care. We thrive to empower our patients and their families to be actively involved in their care process..
You just need to make one call and we put the puzzle pieces together for a seamless patient admission or transition of care.
With all services working together, we created an environment for good communication and effective sharing of knowledge among multidisciplinary providers.
You can get all your needs and your patient needs in one-stop. With our internal coordination of care, the focus is on the total health care needs of the patient.
In our coordination of care, we work to align resources with patient needs. The smooth sharing the information related to patient care among participants can achieve safer and effective care.
Finding a missing piece of the multi-disciplinary healthcare puzzle can be challenging. Our network facilitates the sharing of experience and conflict resolution.
Our internal coordination of care helps us to provide a care that is Safe, Effective, Patient-Centered, Timely, Efficient, and Equitable both within and across systems.
The Joint Commission National Quality Approval rewards Excellence in Leadership and Clinical Care. This continuous quality improvement process is deployed and shared across programs and systems in our network.
We deployed a comprehensive content management EMR system designed to help us optimize our clinical operations across systems and improve the efficiency of our healthcare provision.
