OLIDIA HOME HEALTH FEATURES

Skilled Care, Right at Home.

Olidia Home Health delivers professional, compassionate care in the comfort of each patient’s home. Our team of skilled nurses, therapists, and home health aides provides short-term, intermittent visits focused on recovery, independence, and quality of life.

By combining clinical expertise with a family-centered approach, we ensure every patient receives personalized care that meets their medical, functional, and emotional needs.

SKILLED NURSING

Registered nurses provide intermittent visits for medication management, wound care, IV therapy, and chronic disease monitoring. Their clinical expertise ensures patients receive safe, effective, and evidence-based care at home.

PHYSICAL & OCCUPATIONAL THERAPY

Licensed physical therapists help patients restore mobility, strength, and balance after illness, injury, or surgery. Occupational therapists support patients in regaining daily living skills such as dressing, bathing, and meal preparation. Each program is customized to improve independence and prevent complications.

CHRONIC DISEASE SUPPORT

Our clinicians provide specialized care for conditions such as diabetes, heart failure, COPD, and hypertension. Ongoing monitoring helps reduce hospital readmissions and stabilize health.

MEDICATION MANAGEMENT

Nurses help patients manage prescriptions, dosing schedules, and safe administration. This reduces errors, prevents complications, and promotes adherence to treatment plans.

POST-SURGERY RECOVERY

Home health teams guide patients through rehabilitation after surgeries, helping them regain strength and independence faster. Individualized plans minimize complications and improve healing outcomes.

FAMILY-CENTERED SUPPORT

We partner with families to create a strong, collaborative care plan. This ensures continuity between professional visits and daily home routines, building trust and peace of mind.

A nurse supervisor arrives at your home to perform an initial assessment. This includes your Plan of Care, Medication Profile, Emergency Plan, and any other information that needs to be prepared.

Your nurse will arrive (usually within 24 hours after the initial assessment) to provide hourly care, communicate with your physician when necessary, help show you how to care for your child, and assist you with your other medical care needs.

A representative from the Olidia Care office will contact you and provide you with a nursing schedule, complete with your nurses’ names and the shifts assigned.
