OLIDIA RESIDENTIAL

ADMISSION PROCESS

In our Residential Care Programs, nursing care is only provided by Olidia Care team of highly skilled professionals. We need a minimum of 5 business days to complete the transfer and admission process.
Here are key elements of the process:

 

REFERRAL INITIATION

Olidia receives a referral from a physician, hospital, clinic, or case manager, including basic patient info, diagnosis, and service needs.

 

CARE NEEDS ASSESSMENT

Our clinical team reviews medical records and conducts a patient needs assessment to determine appropriate services and urgency.

 

COORDINATION & SCHEDULING

We assign care coordinators who schedule initial visits, assign field staff, and prepare necessary equipment or services.

PLAN OF CARE DEVELOPMENT

A customized care plan is created by licensed clinicians, approved by the ordering physician, and aligned with payer requirements.

 

PATIENT & FAMILY ORIENTATION

We educate patients and caregivers about their care plan, rights, safety procedures, and how to communicate with the Olidia team.

 

INITIAL SERVICE DELIVERY

Nurses or therapists begin in-home care, documenting electronically in real time and updating the care plan as needed.

 

ONGOING MONITORING & SUPPORT

We provide 24/7 support, adjust care plans as needed, coordinate across services, and ensure compliance and quality assurance.
